A support service coordinator will be assigned to all those who are eligible and wish to receive services.  Their job is to coordinate and monitor the services and supports each person receives, provide information, and refer individuals and families to community resources.  All services offered by The Madison County Board of DD are voluntary.
Ages 3 to 5
If your child is eligible for preschool services, they will be eligible for a service coordinator.  This person’s role is to be a support person, that can attend meetings with you, such as IEP meetings.  They can also help you to find resources and/or additional services that you may need for your child.  The county board does not provide direct services to children ages 3-5, since it is the school districts responsibility to do this.  If you are interested in a service coordinator for your child when they turn three, you must contact Justin Hicks.  He can be reached by email at justin.hicks@madison.oh.gov or by phone at 740-852-7050, EXT. 1848.  He is in the office on Tuesdays and Thursdays.
