QUALIFICATIONS:
SALARY RANGE:
Negotiable based on experience
REGULAR HOURS OF WORK:
40 Flexible Hours, generally 8:00 am to 4:00 pm.
Scheduled as needed to meet the needs of consumers and staff.
IMMEDIATE SUPERVISOR:
Service and Support Administration Supervisor
SPECIFICATIONS:
This is specialized work that involves the planning and monitoring of services for the Home and Community Based Waiver and other service programs that are funded by State and/or local funds. The Service and Support Administrators (SSAs) are responsible for facilitating, coordinating, and continuously reviewing individualized plans for children and adults with developmental disabilities. These plans promote the principles of self-determination and person-centered planning. Job duties may require the availability to deal with urgent situations during non-business hours.
The Service and Support Administrator is responsible for assuring that the services provided for eligible individuals adhere to all regulatory requirements, as well as to Board policy, regulations, and procedures.
JOB DUTIES:
- Bachelor degree preferred, Associate degree required (must work under the supervision of the Service and Support Administration Supervisor).
- Ability to obtain and maintain SSA certification from the Ohio Department of Developmental Disabilities.
- Working knowledge of the SSA rule and other applicable regulations.
- Excellent written and oral communication skills.
- Valid Ohio Drivers’ License with satisfactory driving record.
- Successful completion of a pre-employment physical.
- Satisfactory BCI/FBI, Abuser Registry, Nurse’s Registry, Exclusions List, Sex Offender, U.S. General Services, Medicaid Provider Exclusion & Suspension and Dept. of Rehabilitation and Corrections background checks.

The Service and Support Administrator is responsible for assuring that the services provided for eligible individuals adhere to all regulatory requirements, as well as to Board policy, regulations, and procedures.
JOB DUTIES:
- Completion and review of assessment information and service plans to assure that wants and needs are identified prior to the ending date of each individual service span. Assist in determining annual service budgets for each individual and monitor services on-going.
- Attend team meetings as requested or indicated by the needs of the individual. Provide leadership and facilitate the initial development and revisions to the individual service plan.
- Work with the individual and team members to write measurable and effective supports that identify and develop wants and needs of the person.
- Develop and maintain contact with the individual, their family, and important people in the person’s life.
- Assure Re-determinations for waiver enrollment are completed and submitted prior to the end of each individual’s waiver span. Assist in applying for and maintaining benefits that individuals are eligible to receive.
- Assist in reviewing the needs and requests for service plan revisions including assuring service plans are reviewed and developed annually. Monitor service plan implementation and maintain official record.
- Complete all cost projection information that outlines services for individuals according to the service plan developed. This will include single staffing locations and/or patterns for those individuals that share services
- Provide crisis intervention, as needed, to prevent and/or resolve significant risks to the health and safety of individuals served.
- Serve as liaison between providers of service, recipients, County Board staff, DJFS, DODD, or other relevant agencies. Provide technical assistance as needed to service providers to ensure progression of services occurs and positive outcomes are achieved.
- Provide information to individuals, families, advocates, agencies, and community groups about Medicaid and/or other available services.
- Work with other department personnel on the implementation of processes in support of Medicaid eligible programs.
- Assist with any review or evaluations of the Medicaid Program.
- Provide input on plans of correction for accreditation and provider compliance findings and recommendations.
